Living Wills & Advanced Directive Resource
The Ohio Hospice & Palliative Care Organization,
in cooperation with the Ohio End of Life Collaborative, has produced
Choices: Living Well at the End of Life that contain information and official forms that will help you take control of your future medical care.
The resource contains the official forms:
- The Living Will (LW)
- The Living Will (LW) also contains an “Anatomical Gift” option and Donor Registry Enrollment Form
- The Health Care Power of Attorney (HCPOA)
- An explanation of Do Not Resuscitate Order (DNR)
- An explanation of hospice care and services
This particular packet is for Ohio residents only.
We will have a limited number of copies available the day of the symposium. We recommend
downloading and printing these forms PRIOR to the Symposium and reading them closely.
You will need to have a legal ID with you in order for paperwork to be notarized.
This includes a legal state identity card or passport. Legal name is required for
this paperwork.
